Understanding TB Testing at CVS: What You Should Know

Tuberculosis (TB) is a serious bacterial infection that affects the lungs and can spread from person to person through the air. Testing for TB is one of the first steps in determining whether someone has the disease or has been exposed to it. CVS, one of the largest pharmacy chains in the United States with over 9,800 locations, offers TB testing services at many of their MinuteClinic locations. This guide provides information about what TB tests cost at CVS, how the testing process works, and what factors might affect the price you pay.

TB testing is not something you should delay if you think you've been exposed. The disease can develop slowly, and early detection makes treatment much more straightforward. Many people seek TB tests because they work in healthcare, have been around someone with active TB, or are experiencing symptoms like a persistent cough, chest pain, or unexplained weight loss. Understanding the costs involved before you visit CVS can help you prepare financially and know what to expect during your visit.

CVS MinuClinic locations operate in most states and offer various health services beyond TB testing, including flu shots, physicals, and treatment for common illnesses. However, not every CVS location has a MinuteClinic, so you'll want to check whether one is available near you. The availability of TB testing specifically can vary by location, which is why calling ahead is a practical step before planning your visit.

Practical Takeaway: Before visiting CVS, verify that your nearest location offers TB testing services and learn what insurance or payment methods they accept. Knowing this information beforehand prevents wasted trips and lets you budget accordingly.

TB Test Costs at CVS Without Insurance

The cost of a TB test at CVS MinuteClinic without insurance typically ranges from $40 to $75, depending on your location and the specific type of test performed. Most commonly, CVS offers the Mantoux skin test, also called the tuberculin skin test (TST), which is the traditional method used for decades across the United States. This test involves an injection of a small amount of purified protein derivative (PPD) under the skin, followed by a reading 48 to 72 hours later to see if a reaction has developed.

The cost structure usually breaks down as follows: the initial injection and placement of the test costs one amount, and the follow-up reading (which must happen 2-3 days later) may be charged separately or included in the initial price. Some CVS locations charge around $50 for the initial placement and $25 for the reading, while others bundle these into a single fee. Prices can vary significantly between regions. For example, a TB test in a major metropolitan area like New York City or Los Angeles may cost more than in a rural area, reflecting differences in local healthcare costs.

Without insurance, you pay the full amount out of pocket. This is called the cash price or self-pay price. CVS typically does not negotiate on these prices, though you might ask if they offer any discounts for paying in cash or if they have any current promotions. Some people choose to pay with a credit card that offers cash back or rewards points, which can reduce their net cost slightly.

It's important to note that a TB test is not a single visit. The Mantoux test requires two visits: one for the initial injection and one for the reading. If you cannot return within the 48 to 72-hour window, the test may need to be repeated, which would result in additional charges. Planning these two appointments back-to-back, even if they're not on consecutive days, is essential for a successful test.

Practical Takeaway: Set aside $50-$75 for a complete TB test at CVS without insurance, and mark both appointment dates on your calendar to avoid having to repeat the test due to a missed reading appointment.

TB Test Costs With Insurance Coverage

If you have health insurance, your out-of-pocket cost for a TB test at CVS may be significantly lower. Many insurance plans cover preventive health services, including TB testing, either fully or with a copay. A copay is a fixed amount you pay at the time of service—common amounts are $20, $30, or $50. Some plans may have a coinsurance model instead, where you pay a percentage of the cost (for example, 20%) after you meet your annual deductible.

The type of insurance you have matters significantly. Health insurance plans that comply with the Affordable Care Act (ACA) are required to cover many preventive services without charging a copay or coinsurance when you visit an in-network provider. TB testing may fall under these preventive services in certain situations, particularly if you have a reason for the test (such as exposure to TB or symptoms of illness). However, the coverage rules can be complicated, and not all plans treat TB testing the same way.

To find out what your specific plan covers, you should contact your insurance company directly or check your insurance card and plan documents. You can usually find a phone number on the back of your insurance card. When you call, ask specifically: "Does my plan cover TB testing? Is there a copay? Do I need to meet a deductible first? Will I be charged if I go to CVS MinuteClinic?" Having these answers before you schedule your appointment prevents unexpected bills.

Additionally, verify that your specific CVS location is in-network with your insurance. Going to an out-of-network provider typically means higher costs for you. You can usually check this on your insurance company's website by searching for providers, or you can ask CVS when you call to schedule whether they accept your specific insurance plan.

Medicare and Medicaid coverage for TB testing varies by plan and state. Medicare Part B covers TB testing for beneficiaries with symptoms or exposure, typically at no cost after you've met your annual deductible. Medicaid programs vary by state, but most states cover TB testing for people who qualify for their Medicaid program.

Practical Takeaway: Contact your insurance company before your visit to confirm coverage details, copay amounts, and whether your nearest CVS MinuteClinic is in-network. This single step can save you money and prevent billing surprises.

Types of TB Tests and Associated Costs

Several different TB tests exist, and the type offered may affect the cost. The most common test at CVS is the Mantoux tuberculin skin test (TST), which we've discussed. However, newer blood tests called interferon-gamma release assays (IGRAs) are becoming more widely available. These blood tests are sometimes considered more accurate than the skin test and don't require a second visit for reading. IGRAs include tests like the QuantiFERON-Gold In-Tube or the T-SPOT.TB test.

The Mantoux skin test remains the standard at most CVS locations because it's less expensive to administer and has been used for over a century. The typical cost at CVS is $40-$75 for the complete test (both the placement and the reading). However, some CVS locations may not offer blood-based TB tests like IGRAs, and if they do, the cost is usually higher—potentially $100 to $150 or more—because these tests require laboratory processing.

A chest X-ray may also be recommended if your skin test comes back positive or if you have TB symptoms. A chest X-ray helps determine whether you have active TB disease or simply latent TB infection. CVS MinuClinic locations do not typically perform X-rays on-site, so this would need to be done at a hospital, imaging center, or different medical facility. The cost of a chest X-ray can range from $100 to $300 or more, depending on your location and whether insurance covers it.

Some people may undergo multiple tests if the first test is inconclusive or if they need to retest after exposure. If a Mantoux test is placed incorrectly or the patient misses the reading appointment, the test may need to be repeated entirely, essentially doubling the cost. This is another reason why scheduling both appointments carefully is important.

The specific test recommended to you may depend on factors like your medical history, whether you have symptoms, or local clinic protocols. If cost is a concern, you can ask CVS which type of TB test they offer and at what price when you call to schedule.

Practical Takeaway: Ask which specific TB test CVS offers at your location and whether they have alternatives. If blood tests are available but cost more,
