Understanding Heart Valve Leaks and Treatment Options

What Heart Valve Leaks Are and How They Develop

A heart valve leak, also called regurgitation or insufficiency, happens when one of your heart's four valves doesn't close properly. Your heart has four valves—the mitral, tricuspid, aortic, and pulmonary valves—that act like one-way doors. They open to let blood flow through and then close to prevent blood from flowing backward. When a valve leaks, some blood flows backward instead of moving forward through your heart and body.

Think of it like a door that doesn't seal completely. When you want water to flow in one direction through a pipe, a leaking valve lets some water escape backward. The same principle applies to your heart. Each time your heart beats, the chambers contract to pump blood out. If a valve doesn't close tightly, blood that should move forward gets pushed back.

Heart valve leaks can develop for several reasons. Some people are born with valve problems. Others develop leaks over time from wear and tear, infection, or disease. The mitral valve, located between your heart's left upper and lower chambers, is the most commonly affected. The aortic valve, which controls blood flow from your heart to your body, is the second most common site of leaks.

According to the American Heart Association, valve disease affects millions of people in the United States. Some valve leaks are small and cause no problems. Others are severe and require treatment. The severity depends on how much blood leaks backward and how well your heart compensates.

Takeaway: Heart valve leaks occur when valves don't close properly, allowing blood to flow backward. Understanding which valve is affected and how severe the leak is helps determine what monitoring or treatment may be needed.

Common Causes of Valve Leaks

Multiple conditions can cause heart valve leaks. One major cause is rheumatic fever, a complication from untreated strep throat. When strep infections go untreated, they can inflame the heart and permanently damage valves. While rheumatic fever is less common in developed countries due to antibiotics, it remains a leading cause of valve disease worldwide. The World Health Organization estimates that 40 million people live with rheumatic heart disease globally.

Endocarditis, an infection of the heart's inner lining, can also damage valves. This infection can develop when bacteria enter the bloodstream through dental work, surgery, or injection drug use. The infection attacks the valve tissue and can create holes or prevent the valve from closing properly. People with existing valve problems, weakened immune systems, or heart defects face higher risk.

Age-related wear is another common cause. Over decades, valves naturally experience stress from billions of heartbeats. The tissue can become thickened, stiff, or calcified—a process where calcium deposits build up on the valve. This is especially true for the aortic valve. Studies show that aortic valve disease affects about 2% of people over age 65 and increases to nearly 10% of those over 85.

Heart conditions like dilated cardiomyopathy can also cause leaks. In this condition, the heart muscle weakens and enlarges. When the heart chamber becomes too large, the valve leaflets can't meet properly, creating a gap that allows leakage. High blood pressure, prior heart attacks, and certain viral infections can lead to cardiomyopathy.

Other causes include:

• Heart attacks that damage valve support structures
• Trauma or chest injuries
• Connective tissue disorders like Marfan syndrome or Ehlers-Danlos syndrome
• Certain medications or radiation therapy to the chest
• Congenital (present at birth) valve abnormalities
• Atrial fibrillation, an irregular heart rhythm

Takeaway: Heart valve leaks develop from diverse causes including infections, aging, heart disease, and congenital conditions. Identifying the underlying cause helps your doctor recommend appropriate monitoring and treatment options.

Recognizing Symptoms and When to Seek Medical Evaluation

Many people with mild valve leaks experience no symptoms at all. The heart compensates for the extra work, and daily activities proceed normally. However, as a leak becomes more severe, symptoms often appear. Recognizing these signs can help you know when to see a doctor.

One common symptom is shortness of breath, especially during physical activity or when lying flat. Your lungs can become congested with fluid when the heart can't pump blood forward efficiently. You might notice you get winded more easily than before or can't climb stairs or exercise as long as you used to. Some people wake at night feeling short of breath.

Fatigue is another frequent symptom. Because your heart is working harder to move blood forward despite the leak, you may feel unusually tired. This fatigue doesn't improve with rest and can affect your daily activities. You might feel weak or notice decreased stamina throughout the day.

Heart palpitations—feeling like your heart is racing, skipping beats, or pounding—can occur. You might notice these sensations in your chest, neck, or throat. While palpitations can be harmless, they warrant medical evaluation to rule out serious conditions. Some people describe the sensation as a flutter or thump.

Other symptoms that may indicate a valve problem include:

• Swelling in your ankles, feet, or legs
• Bloating or fullness in your belly
• Dizziness or lightheadedness
• Chest discomfort or pain, especially with exertion
• A heart murmur detected during a physical exam
• Irregular heartbeat or arrhythmia

You should contact a healthcare provider if you experience any of these symptoms, especially if they're new or worsening. Don't ignore persistent shortness of breath or chest discomfort. Your doctor will listen to your heart and may recommend tests to evaluate valve function.

Takeaway: While mild valve leaks often cause no symptoms, more severe leaks may cause shortness of breath, fatigue, palpitations, or swelling. Report any new or worsening symptoms to your healthcare provider for evaluation.

How Doctors Diagnose Valve Leaks

Diagnosis begins with a medical history and physical examination. Your doctor will ask about symptoms, family history of heart disease, past infections like rheumatic fever or endocarditis, and any medications you're taking. During the physical exam, your doctor uses a stethoscope to listen to your heart. A leaking valve often produces a heart murmur—an extra sound or whooshing noise that indicates abnormal blood flow. Not all murmurs mean serious disease, but murmurs warrant further investigation.

Echocardiography is the primary test for diagnosing valve leaks. This ultrasound creates moving images of your heart as it beats. The test shows the size and shape of your heart chambers, how well the valves open and close, and how much blood leaks backward. Doctors can measure the severity of the leak and assess how your heart is functioning overall. An echocardiogram is non-invasive, uses no radiation, and takes about 20-40 minutes. This test provides detailed information that guides treatment decisions.

Doppler ultrasound is often combined with standard echocardiography. Doppler uses sound waves to measure the speed and direction of blood flow. It shows precisely where blood is leaking and how severe the leak is. Colors on the ultrasound screen indicate the direction and velocity of blood movement, making valve problems visible.

Your doctor may also order other tests depending on findings:

• Electrocardiogram (EKG) to check for irregular heart rhythms or heart strain
• Chest X-ray to see the size and shape of your heart and check for fluid in the lungs
• Stress testing to see how your heart responds to exercise
• Cardiac CT scan to get detailed images of your heart structure
• Cardiac MRI for more detailed images when other tests are unclear
• Cardiac catheterization, a procedure where a thin tube is threaded
